We are currently seeking a Part-Time Licensed Social Worker (LBSW) to enrich our team.

The Social Worker (BSW) plays a crucial role within our interdisciplinary team, primarily focused on providing vital psychosocial support to the patient/family unit. You will conduct assessments to identify needs, goals, and potential interventions as part of the established plan of care, utilizing your professional expertise and personal judgment to monitor the psychosocial assessment process.

Key Qualifications:

• Bachelor's degree in social work from a CSWE accredited School.
• Licensure as mandated by state regulations.
• A minimum of one year experience as a Social Worker in healthcare and/or hospice settings is required; three to five years is preferred.
• Understanding the dynamics surrounding terminal illness, loss, and death, along with the unique needs of hospice patients and families.
• A compassionate approach and tact in dealing with patients facing crises and the ability to manage the stress of repeated loss.
